Ready for Paintball?

Jr. High & Sr. High, you're invited to join us for an afternoon of paintball at Wasaga Adventure Park!

When: Tuesday, August 5 | 12:15pm
Where: Wasaga Adventure Park | Meet at Cornerstone at 12:15pm for a pizza lunch. Bus departure at 1pm.
Cost: $50/person

How To Sign Up:

If You're 18 years of Age or Older: 

Step 1: Complete the online Registration & Waiver form below.
Step 2: Submit your payment: the cost is $50/person, and payment options are listed below. Payments must be made prior to the event.

If You're Under 18 years of Age: 

Step 1: Your parent/guardian can complete the online Youth Registration & Waiver form below.
Step 2: Submit your payment: the cost is $50/person, and payment options are listed below. Payments must be made prior to the event.

Make A Payment:

The cost to attend is $50 per person; you can pay through the following options:

1. E-Transfer: You can e-transfer your payment to give@cornerstoneorillia.org.  Please ensure you write in the memo line that your payment is for paintball, and the name(s) of the participants.

2. PayPal: You can click the PayPal button below to send your payment; you don't need a PayPal account to use this option, PayPal accepts all major credit/debit cards (just click 'don't have an account' on the screen after clicking the PayPal button below)

3. Cash/Cheque: You can bring your payment to Youth before July 31.

Indemnity Waiver:

Precautions are taken for the safety and health of all those attending off site events/activities organized by Cornerstone Baptist Church, Orillia (the “Church”) however, in the event of an accident or sickness, the undersigned hereby agrees to indemnify and hold harmless Cornerstone Baptist Church, Orillia, the Board of Elders, its staff and its volunteers from and against any loss, damage or injury suffered by me while participating in or travelling to events/activities organized by the Church.

Information received is confidential and is being gathered for the purposes of serving your student while in the care of Cornerstone Baptist Church, Orillia. Any medical information collected here serves to authorize Cornerstone Baptist Church, Orillia and its staff and volunteers, to obtain medical assistance in emergencies.
PLEASE NOTE: In the case of custody agreements, include the proper form authorizing parental contacts.

SECTION 1

Please complete the following information. The safety of your youth is our primary concern. Precautions will be taken for their wellbeing and protection.

If your information has not changed, you can leave the rest of Section 1 blank and move to Section 2 and 3

Purposes and Extent: Cornerstone Baptist Church, Orillia is collecting and retaining this personal information for the purpose of enrolling your student in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your student, and to inform you of program updates and upcoming opportunities at our Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Cornerstone Baptist Church to limit the information collected, or to view your student’s information, please contact us.

SECTION 2

Please complete each waiver below by selecting 'I agree' or 'I disagree'.

I/we, the parents or guardians named above, authorize Josh Scott, Noah Ecker or one of the Cornerstone Baptist Church, Orillia Ministry Personnel, to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.
I/we, the parents or guardians named above, undertake and agree to indemnify and hold blameless Cornerstone Baptist Church, Orillia, the Board of Elected Elders and Ministry Personnel from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Cornerstone Baptist Church, Orillia as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in or traveling to or from events of Cornerstone Baptist Church, Orillia.
I/we, the parents or guardians named above, GRANT PERMISSION for the reasonable use of pictures containing our student in any or all of the following ways (please indicate any or all):
