As we are going on off-site trips in the afternoons, all volunteers are required to complete a liability waiver. Please complete the form below in full; if you are under 18, please complete the Under-18 Volunteer Waiver here instead. All information is kept confidential and sent to the CM team for their files. Precautions are taken for the safety and health of all those attending off site events/activities organized by Cornerstone Baptist Church, Orillia (the “Church”) however, in the event of an accident or sickness, the undersigned hereby agrees to indemnify and hold harmless Cornerstone Baptist Church, Orillia, the Board of Elders, its staff and its volunteers from and against any loss, damage or injury suffered by me while participating in or travelling to events/activities organized by the Church.
